What’s MI Health Link? 

MI Health Link is a program that offers coordinated medical, mental health, and substance use disorder services for residents in Macomb County who are covered by both Medicare and Medicaid.

 

Am I Eligible?

The MI Health Link Program is available to those who meet the following criteria: 

  • Live in Macomb County
  • Have Medicare Part A, Part B, and Part D
  • Are eligible for full Michigan Medicaid benefits (without a “spend down” or deductible)
  • Are not enrolled in hospice care
  • Are not enrolled in the MI Choice Waiver Program or the Program of All-Inclusive Care for the Elderly (PACE)
    • If you are currently enrolled in either of these programs, you must disenroll before you can register for the MI Health Link Program

What are the advantages of the MI Health Link program?

As an MI Health Link member, your physical and behavioral health services will be fully covered—including your prescriptions. You will not have to pay extra money to join this health plan. MCCMH and your health plan will work to make your Medicare and Michigan Medicaid benefits work better for you.

You will not pay a deductible or copayment when you receive services from a provider or pharmacy in the plan’s provider network. You will have your own Care Coordinator or Care Manager who will ask about your healthcare needs and choices and work with you to create a personal healthcare plan based on your goals. You will be able to address both medical and behavioral health issues at the same time.

Who should you contact regarding MI Health Link services?

For specific questions about your MI Health Link services at MCCMH, please call the Customer Service department at 855-99-MCCMH (855-996-2264).

For questions about Medicaid benefits, call the Medicaid Beneficiary Helpline at: 1-800-642-3195.

For questions about Medicare benefits, call Medicare at 1-800-MEDICARE (1-800-633-4227), 24 hours a day, seven days a week. TTY users should call 1-877-486-2048. 

For questions about Medicare and Medicaid in Michigan, you can contact the Michigan Medicare, Medicaid Assistance Program (MMAP) by dialing 1-800-803-7174.
