A Substance Use Disorder Health Home (SUDHH) is not a physical location, but a program designed to address your health and social needs. Individuals in recovery should prioritize their overall well-being, as these factors significantly influence daily life, learning, and the ability to work effectively.

A SUDHH can help you in achieving your recovery goals by offering a personalized treatment plan for substance use, along with support for your mental and physical health. Services may include:

  • Extensive care management  
  • Care coordination 
  • Health promotion 
  • Comprehensive transitional care 
  • Individual and family support 
  • Referral to community and social support services

You may be eligible for the SUDHH if:

  • You are diagnosed with an opioid use disorder, alcohol use disorder, or stimulant use disorder 
  • You have Medicaid, Healthy Michigan Plan, Freedom to Work, or Healthy Kids Expansion
